Managing diabetes daily is a challenge that sometimes can be overwhelming. Lutheran HealthCare offers several options to help community members control their diabetes and stay healthy.

The Diabetes Self-Management Education Program is recognized by the American Diabetes Association as meeting the national standards for patient diabetes self-management education. The program offers a series of four group education classes in diabetes management on a monthly basis at the Sunset Park Family Health Center, Brooklyn, and on a bi-monthly basis at other clinic sites. The program also provides nutrition counseling, blood glucose meter teaching and case management by nursing staff.

Gestational Diabetes Management

This program supports Women's Health Services, particularly the perinatology division, by providing education and case management to pregnant women with gestational diabetes. The clinical goal is to reduce the morbidity and mortality associated with uncontrolled blood sugar during pregnancy. A Blood Glucose Meter Loaner Program was implemented to assist those patients who are in the Prenatal Care Assistance Program (PCAP) and do not receive Medicaid.

Community Support Group

For many people with diabetes, peer-to-peer interaction provides a positive and effective way to discuss issues, share concerns and learn from one another. The Diabetes Self-Management Education Program offers a monthly community support group. It is designed to provide a supportive environment in which people with diabetes can meet and share coping strategies, successes and other issues. The support group does not provide formal diabetes education or clinical intervention.

Weight Management

The Latino population is one of the largest minority groups in the United States, representing 13 percent of the population, according to the 2000 U.S. Census. It is also a group at high risk for diabetes in our community. Open to all, the Lutheran Family Health Centers provide a Family Weight Management Program, targeting weight control and diabetes prevention in this community. The curriculum is structured as an outpatient weight-management and exercise program, delivered in the following two phases:
  • 16 weeks of exercise and education
  • 16 weeks of maintenance activities that promote long-term compliance with lifestyle changes learned in the first phase of the program.

Diabetes "Lunch 'N' Learn"

This program offers educational opportunities to health care providers at all Lutheran HealthCare sites in the form of quarterly Lunch 'N' Learn sessions. The program also provides education and support to the nurses in the school health program.

Patients may be referred to the Diabetes Self-Management Education Program through their primary care providers, but a referral is not necessary.
