The Asthma Management Program offers Lutheran HealthCare patients a series of individual self-management educational sessions, targeted to each individual's or family's needs. Sessions include basic asthma information about signs and symptoms, control of environmental triggers, use of medications and use of durable medical equipment. This information helps patients link symptoms with medication use for better asthma control. Nurse case managers and health educators work with patients' primary care providers and pulmonologists to adjust the treatment plan to each patient, and physician-team conferencing and management of care by professional nursing staff are some of the interventions that the program can help patients to arrange. Patients may be referred to the program by their primary care providers, but referral is not necessary.

The Smoking Cessation Program offers three to five consecutive sessions that help participants learn behavioral modification skills and offer psychological evaluation on the issue of tobacco use. A team approach, among the patient, health care provider, and counselor, is incorporated to guarantee optimal results. The program allows for walk-ins and referred patients. The program also provides staff development training for health care professionals and laypersons.
