Many of our neighbors are older adults and creating the right programs to meet their needs has been a critical focus for Lutheran HealthCare. To help support our neighbors throughout all stages of their life, we have developed a full array of services for older adults that includes wellness activities and health education; housing and supportive services; access to medical and rehabilitative care; social adult day programs; providing meals to those who need them; and long-term care. These services are apart of Lutheran HealthCare's commitment to helping community members stay healthy and active as they age.

Programs
Service Coordination helps seniors in our housing sites continue living independently. The Service Coordinators provide information and referral, advocacy services, and assistance working with building management around issues that may affect a tenant's ability to age in place.

Adult Day Program - Seniors in Touch is a social adult day program in the Bay Ridge and Sunset Park communities. Seniors in touch operates out of two sites – Shore Hill Housing on Monday, Wednesdays and Fridays and Harbor Hill Housing on Tuesdays and Thursdays from the hours of 10 a.m. –3 p.m. The goal of the programs is to provide arts oriented activities to seniors who are frail and socially isolated. Yoga, tai Chi and gentle exercises are part of the program. This program fosters independence and socialization. Lunch is provided by St. John's/Fort Hamilton Nutrition program and by the Sunset Park Senior Citizen Center. Transportation is provided by the program through the use of a handicapped-accessible paratransit vehicle. Nominal participant contribution is requested.

Transportation Services - Bay Ridge on the Move isa fixed-route transportation service providing seniors with easier access to neighborhood shopping, medical appointments, visits with family and friends. For more information (click here).

Wellness Services - Health Accent is a membership program providing adults 55 and older with activities to help them remain healthy and active. Services include a walking group, a swimming group, educational and recreational trips, health and wellness presentations and a quarterly newsletter.

Care Giver Support Services - Weekly support groups and educational workshops are offered for individuals caring for older adults in the community.

Full Circle - specializes in the care of older adults with Alzheimer's disease and other forms of dementia. Participants enjoy a safe and pleasurable day of mind-stimulating activities and gentle exercise while caregivers benefit from needed respite support.
