The Park Slope Family Health Center, which opened in 1990, is a beautiful, modern health care facility conveniently located on 13th Street between 4th and 5th Avenues in Park Slope.

For new patients, please arrive 30 minutes prior to your appointment, all others arrive 15 minutes prior to appointment.

Tuesday through Thursday, 8:30 a.m. - 8 p.m.
Saturday, 8:30 a.m. - 4:30 p.m.
