    • In 2009, The Lutheran Family Health Centers, the outpatient arm of Lutheran HealthCare, served 131,741 patients from the ethnically diverse, medically underserved neighborhoods of southwest and central Brooklyn. The network, which is governed by the Sunset Park Health Council, Inc., includes eight primary care sites, twenty - eight school based health clinics and dental clinics and numerous social support services. The Lutheran Family Health Centers network has grown to be one of the largest, most comprehensive Section 330-funded Federally Qualified Health Center (FQHC) networks in the nation.
    • Community-based services , including Adult and Family Education; Healthy Connections; AmeriCorps Community Healthcorps; Women, Infants and Children Program (WIC); Reach Out and Read; and Chronic Disease Management 

  • Lutheran Augustana Center for Extended Care and Rehabilitation, is a 240-bed extended care and rehabilitation facility providing 24-hours-a-day skilled nursing, medical and support services for those requiring both short-term restorative rehabilitation and/or long-term care. Augustana is recognized for its intensive short-term restorative rehabilitation program for the post-hospital treatment of disabilities caused by complex orthopedic, neurological and other acute or chronic medical conditions. Other clinical services include individualized nutritional care, social services, hospice and pastoral care. In addition, Augustana operates a long-term home health care program to coordinate comprehensive medical, nursing and support services in the comfort of one's own home.

  • Harbor Hill, Shore Hill and Sunset Gardens Subsidized Senior Housing developments, providing seniors with more than 725 federally subsidized, independent apartments as well as, access to medical care, social activities, meals for those in need, health education, wellness activates and support services.

  • Community Care Organization, Inc., established in 1984 as a not-for-profit home care program, the Community Care Organization is licensed by the N.Y.S. Department of Health and accredited by the Joint Commission. Community Care provides high quality in-home personal care by state certified home health aides and support services to the frail elderly, handicapped and other convalescing individuals.

Providing a three-pronged approach with three distinct levels of care: inpatient rehabilitative hospital care, short- and long-term sub acute care and three outpatient facilities
