Social work services are provided to hospitalized patients and their families in planning for post hospital care. The social workers collaborate with the other members of the health team, and the patient/family in developing a discharge plan. This could include home care services, short-term rehabilitation, and long-term placement in a skilled nursing facility, or referral to community based programs. Social work counseling related to the hospitalization or the illness and its effect on the patient and family is also provided.
